1.  Application in Form-PTA   / PSPLA          
2. Registration Certificate
3. Insurance Certificate
4. Primary Permit
5. Valid Tax
6. Emission test certificate

         
3.     What is the amount of Fees payable?

       Rs.150/-

               Service Charge Rs.50
               Total Fees Rs.200/-
