0RDER:

A workshop on Integrated Disease Surveillance Project was held at New Delhi on 3rd – 5th April 2003. The nodal officers of all States of Phase-1 under Integrated Disease Surveillance Project and the officials of Government of India and other institutions of health field participated in the above workshop. One of the important items decided at the workshop was to have a State Executive Committee at State level and District Executive Committees at District level for proper implementation of Integrated Disease Surveillance Project to have close monitoring of the project activities. The workshop also indicated that the State Executive Committee might be chaired by the Director of Health Services and District level Committee by the Chief Medical Officers.

2. Accordingly, the Director of Public Health and Preventive Medicine has requested that State Level Executive Committee and District Level Executive Committee may be formed.

3. The Government, after careful examination accept the request of the Director of Public Health and Preventive Medicine and constitute the State Level Executive Committee with the following officials:
