Abstract Establishment – Outsourcing of Hospital Workers and Sanitation staff in Government Hospitals Under the control of Director of Medical Education and Director of Medical and Rural Health Services – Orders – Issued. Health and Family Welfare (E1) Department G.O.Ms.No.103 Read Again:
Order: In the G.O first read above orders have been issued for privatising the areas of Security, Catering and Sanitation in Government Hospitals. The Director of Medical Education has requested Government orders on terms and conditions to be adopted for outsourcing the above services in Government Hospitals. 2. The methodology to be adopted for outsourcing the Hospital Workers and sanitation staff in Government Hospitals under the control of the Director of Medical Education & Director of Medical and Rural Health Services was examined by Government and they pass the following orders:- a) (i) The staff strength of Sanitary Workers and Sweepers under the Control of Director of Medical Education is fixed at the ratio of one worker for every 12 beds. (ii) The staff strength of Hospital Workers under the control of Director of Medical Education is fixed at the ratio of 1 worker for every 6 beds. (iii) As Male Nursing Assistant, Grade II are carrying out slightly different functions in teaching hospitals and merging them would adversely affect the functioning of the Hospital, a policy for outsourcing the vacant posts of Male Nursing Assistant will be worked out separately. Till then, the vacant posts of Male Nursing Assistant Grade-II in the Director of Medical Education side should be kept unfilled. b) (i) The staff strength of Sanitary Workers and Sweepers under the control of Director of Medical and Rural Health Services is fixed at the ratio of one worker for every 16 beds subject to a minimum of 3 posts for a less than 30 bedded Government Hospital. (ii) The staff strength of Hospital Workers under the control of Director of Medical and Rural Health Services is fixed at the ratio of 1 worker for every 8 beds subject to a minimum of 3 posts for less than 30 bedded Government Hospitals. (iii) The revised ratio requires an increase of 925 Hospital Workers to Director of Medical and Rural Health Services side. As on date 956 Male Nursing Assistant Grade II are in position against the sanctioned posts of 1311. The pay scales of these two categories are identical. It is therefore ordered that these 956 Male Nursing Assistants Grade II be clubbed with Hospital Workers in the Director of Medical and Rural Health Services side for working out the eligibility as per the ratio, leaving the 355 posts vacant. As and when the posts fall vacant in Male Nursing Assistant II category in future in the Director of Medical and Rural Health Services side they will be treated as Hospital Workers and filled up by outsourcing. c) After arriving at the staff strength to each Hospital /Medical Institutions as per the above norms, the vacancies to be filled up will be assessed against the posts as arrived at by following the norms or the sanctioned posts whichever is less. Illustratively if there are 300 beds in an Institution in Director of Medical Education / the Institution would be eligible for 50 posts of Hospital Workers and 25 posts of Sweeper and Scavengers i.e. totally 75 posts in these categories. Even if there is a sanctioned of 75 posts of Hospital Workers and 30 posts of Sweepers and Scavengers, vacancies will be filled only when they fall below the normative level, i.e., when they fall below the normative total strength of 75 posts. Alternatively if there are only 60 posts of Sweepers / Scavengers and Hospital Workers sanctioned to that institution, vacant posts can be reckoned against the 60 sanctioned posts only. d) The Government direct that the total emoluments of the vacant posts should be calculated at the minimum of scale discounted by a factor of 30%. This should be the upper limit against which the value of tender for outsourcing these services shall be decided. e) The categories, which do not fall under the 63 clubbed categories of Hospital Workers but fall under Basic Service should also be outsourced by calling for a tender by following the same formula i.e. Vacant posts x Emoluments at minimum of time scale minus 30%. 3. The Director of Medical Education and Director of Medical and Rural Health Services are requested to send proposals to Government for opening of a new head of account under salary head for provision of funds and till a new head of account is opened, expenditure towards payments to services contractors shall be booked either under “wages” or “payment for professional and special services” under the respective service head of account. 4. All Deans of Medical College Hospitals / Superintendents of Medical Colleges attached Hospitals / Institutions, District Head Quarters Hospitals, Medical Officers of Taluk Hospital are permitted to incur the expenditure pending provisions of funds in RE / FMA 2003-2004 by reappropriation. Pay and Accounts Officer / Treasury Officers are requested to admit the bills if otherwise found to be in order. The expenditure should be included in the RE/FMA proposals for 2003-2004 at the appropriate time. 5. The above system shall be reviewed at the end of one year for streamlining further. These orders take immediate effect. The Director of Medical Education / Director of Medical and Rural Health Services are requested to communicate a copy of this order to their Subordinate Officers for Compliance and implementation.
6. The regular staff and contract employees should not be posted for work in the same buildings and instead they should be posted in the separate buildings as already ordered in para 3 (i) of G.O.Ms.No.4 Health and Family Welfare Department dated 7.1.2002.
7. This order issue with the concurrence of Finance Department vide its U.O.No.93/ADS (KSG) 03-1 dated 13.5.2003.
